Instructions for Application

1. Save a copy of the Application form
2. Open the saved application in Adobe Acrobat Reader or a PDF viewer/editor of your choice
3. Fill out the form by typing text directly into the form and clicking in the appropriate check boxes
4. Save the filled-out application as a PDF and send by email to LME

If you are unable to save it as a PDF, print the filled-out application and mail, fax, or submit it in person to the LME office in Anaheim using the mailing address and fax number given in the application. If you fax the application, send an email to LME to notify us of this (in case the fax is not received).

If you are unable to fill out the application by typing text directly into the form, print the blank application, fill it out by hand, and after making a copy for your records, mail, fax, or submit it in person to the LME office in Anaheim using the mailing address and fax number given in the application. If you fax the application, send an email to LME to notify us of this (in case the fax is not received). If you are able to scan the application (preferably to PDF), send it to LME via email.

Be sure to save or print a copy of your application for your records.
